Abstract
Background: This study aimed to investigate the relationship between estimated glomerular filtration rate (eGFR [mL/min per 1.73 m2]) measuredusing modification of diet in renal disease equation and hand-grip strength (HGS [kg]) in patients ¡Ã60 years of age in South Korea.

Methods: Our analysis involved 5,165 individuals from the Korea National Health and Nutrition Examination Survey 2016 to 2018. The participants wereaged ¡Ã60 years, and their HGS and serum creatinine levels were measured. Participants diagnosed with cancer were excluded. The participants werecategorized according to their eGFR: subgroup 1, eGFR¡Ã90; subgroup 2, 60¡ÂeGFR<90; subgroup 3, 45¡ÂeGFR<60; and subgroup 4, eGFR<45. Theparticipants were also categorized into the stronger and weaker HGS groups based on their average HGS. Data were analyzed using Pearson¡¯s chisquared test and logistic regression analysis.

Results: After correcting all confounding variable, decreased kidney function was associated with increasing odd ratios for weak HGS in femaleparticipant (model 3, P for trend=0.043). Additionally, subgroup 4 (eGFR <45 mL/min per 1.73 m2) showed that the odd ratio for weak HSG was4.496 (95% CI, 1.422?14.211; P for trend=0.043), when compared to subgroup 1 (eGFR ¡Ã90 mL/min per 1.73 m2) and the stronger HGS group infemale participants; however, it was not statistically significant in male participants.

Conclusion: Among the elderly (¡Ã60 years) in South Korean, female patients with an eGFR <45 mL/min per 1.73 m2exhibit reduced HGS. Futureresearch should focus on clarifying the relationship between eGFR and HGS.
